Use of Services

Flores Therapy and Wellness provides mental health therapy services to clients residing in Arizona and Virginia. Services are provided by clinicians licensed in Arizona and/or Virginia, and clients must reside in a state where the provider holds licensure. By using our services, you confirm that you meet this residency requirement.

Not Medical or Crisis Services

Our services do not replace medical or psychiatric treatment. If you are experiencing an emergency, call 911 or go to the nearest emergency room.

Intellectual Property

All content on this site is the intellectual property of Flores Therapy and Wellness and may not be reproduced without permission.

No Guarantee of Outcomes

Therapy outcomes vary based on individual factors. We do not guarantee specific results.

Scheduling and Cancellation

Appointments may be canceled or rescheduled with at least 24 hours' notice. Missed appointments or late cancellations may result in a $150 fee.

Fees and Payment

You agree to pay all fees associated with services rendered. We accept credit/debit cards, HSA cards, or other electronic payment methods through our secure client portal. Sliding scale options may be available on a case-by-case basis.

Insurance

We may be in-network with certain insurance providers. It is your responsibility to verify coverage and benefits. Submission of superbills for reimbursement is the client's responsibility. Flores Therapy and Wellness makes no guarantees regarding reimbursement.

Telehealth Services

Telehealth services are delivered in compliance with applicable laws and licensing board regulations. Clients must be physically located in
